# If Your Card is Lost, Stolen, or Damaged

> Contact your Administrator or PEX immediately to report lost, stolen, or damaged cards, as well as for any fraudulent charges.

Only PEX can issue replacement cards. Administrators may create only new cards.

**Lost/Stolen** card replacements will immediately close all prior cards on the account once the shipping address and shipping method have been confirmed. This is a permanent action.

**Damaged** card replacements will allow the previous card to remain active until the replacement arrives in the mail. Activating a replacement card will immediately close all previous cards on the card account.

*Cardholders* may call **866-685-0898**, available 24/7, for a replacement to the address on file with First Class FREE (10-15 business days) shipping only.

For a change of address or shipping method, only Administrators may request Priority - **\$10** (4-8 business days), Expedited- **\$55** (5 business days) or Rushed - **\$70** (4 business days) delivery of replacement cards. 

*Administrators* may call **866-685-1898**, available 24/7, for a replacement to any domestic address within the 50 states, and with the shipping method preferred.
